I'm surprised that the bottle of 'rotgut' that Captain Hazelwood drank on the night of March 2, 1989 isn't at the top.

A fine of $50,000 plus 1,000 hours community service, plus a complete loss of his career? Yeah, that put him on up there, probably in third place or so. That's pretty expensive for a wreck that occurred while he was off the bridge, and those on the bridge failed to carry out his orders.
