Can't say for certain, but if I wanted to set it up, I'd have a tube up my sleeve and palmed in the hand that I used to operate the "tap."  The handle would be hollow and open down through the "tap" - I'd stick the tube into the hollow handle when I grabbed it.  I'd control the flow with a foot pedal.
