It's a form you fill out along with some extortion money to get the plates, to lessen the likelihood of being stopped by the pigs.

    The temple?  I havent visited  since before I had facial hair, and my modem
was connected to the temples BBS at a whopping 2400 bps, and paying long distance
fees. Its been awhile.
